Can Cirrhosis be treated ?
Cirrhosis is an irreversible and progressive disease. There is currently no treatment for cirrhosis.
Regular treatment is based only on giving symptomatic relief to the patient. This includes medicines for acidity, limb swelling, abdominal fluid, multivitamins and control of upper g-i bleeding . However, none of these medicines can cure cirrhosis or stop its progression.
Liver transplant is universally the standard of care for treatment of End-stage liver disease (Cirrhosis).
How do doctors assess the severity of cirrhosis ?
There are several scoring systems that can be used for the assessment of severity of cirrhosis. These include the CTP (Child-Pugh-Turcotte) score and the Model For End Stage Liver Disease (MELD) score. Patients with early liver disease have minimal symptoms, low scores and they don’t need a transplant. Patients with advanced disease have severe symptoms, high scores and need early transplant. Patients who have secondary effects of liver disease on other organ systems such as kidney (Hepato-renal syndrome), lungs (Hepato-pulmonary syndrome) or brain may also need an early transplant.
When is Liver Transplantation indicated in a cirrhotic patient ?
All patients with CTP score of more than 8 and a MELD score of more than 15, and who are severely decompensated (bloody vomiting, fluid in abdomen, jaundice, swelling of the lower limbs, alteration in sensorium) are candidates for liver transplantation. Patients in whom the disease has progressed and is now affecting other organ systems (kidney, lungs, heart) will also require urgent liver transplant.
